  • How well did the candidate work with others?

  • What factors are contributing to the candidate's departure?

  • How does the candidate handle stress?

  • How would you rate the candidate's listening skills and ability to receive constructive feedback?

  • How was the candidate's attendance and punctuality?

  • If you could give any advice to the candidate's next manager, what would it be?

  • Would you rehire the candidate? Why or why not?

  • The candidate is under consideration for a           position. Can you recommend her/him for this type of job without any reservation?
